Effective January 1, 2011, FMLA requests, in addition to Short Term Disability requests, will be administered through Unum - see Forms section below. Contact Unum directly at 866.779.1054. Spectrum Healthcare Resources employees will continue to call Spectrum Human Resources for FMLA and will call Unum for Short Term Disability.
