United Disease Management
Employees and dependents with United health insurance who have chronic conditions such as diabetes, coronary artery disease, asthma, congestive heart failure, and COPD may self-enroll in United's disease management program by calling 866.675.1608. This program is completely free, and it is customized to meet individual needs.

Personal Health Risk and Diabetes Coaching
The LiveWell Program also offers free personal health risk and diabetes coaching for employees that have United health insurance. Click here for more information and to download an application!

Prescription Information
TeamHealth employees and dependents covered under a non-High Deductible Plan pay 35% of retail and 25% of mail order for prescription drugs. If you have a maintenance medication, you may save considerable amounts by ordering through mail order. Additionally, all diabetic, asthma, and cardiac drugs are subsidized by TeamHealth at a higher rate. Participants only pay 20% of mail order for drugs that fall into these classes.
