Health Risk Assessment

One of the best ways to avoid health complications is to know your risk. All employees and spouses with TeamHeath medical insurance are encouraged to complete an annual health risk assessment.

Have you completed the health risk assessment? Employees and spouses with TeamHealth medical insurance through UnitedHealthcare who complete a brief online health risk assessment with biometric values (body weight, blood pressure, cholesterol) between July 1, 2011, and March 31, 2012, will each be awarded a $50 credit towards health insurance. To complete the brief health risk assessment, go to www.MyUHC.com (click “Health Assessment” once you are logged in). If you have not registered, click “Register Now” (use the Member ID and Group Number found on your UnitedHealthcare insurance card).
