Currently, incentives are available for employees who participate in the LiveWell Program. The following are offered by LiveWell:

Program Incentive
Health Risk Assessment $50 health insurance credit for taking a brief 15-minute online assessment
Telephonic Health Coaching

Free telephonic health coaching for weight loss/maintenance, exercise, stress management, tobacco cessation (available to all United participants)

Exercise
  • $50 quarterly health insurance credit for exercising at least 30 minutes, 3 times a week
  • Local fitness facility discounts through GlobalFit.com/TeamHealth
Nutrition and Weight Management
  • Free individualized health coaching with a nurse for employees who have TeamHealth medical insurance and a BMI>30
  • $25 health insurance credit for each BMI reduction
  • $50 credit for joining a weight loss program (e.g., Weight Watchers)
  • $50 credit for meeting your goal with that program
Tobacco Cessation
  • $50 credit for completing a cessation program
  • Reimbursement of certain prescription drugs for employees with TeamHealth medical insurance
Healthy Pregnancy $50 Babies R’ Us gift card for enrolling in the healthy baby program and another $75 gift card for program completion
Disease Management
  • Free individualized diabetes coaching with a nurse for employees on TeamHealth insurance
  • Disease management maintenance medications subject to 20% coinsurance instead of the normal 25% coinsurance
  • $50 quarterly credit towards health insurance for compliance with UnitedHealthcare’s disease management program
Mental Health
  • Free 24/7 access to the Employee Assistance Program (EAP) for all employees
  • First three in-person visits to a mental health professional covered at 100% if referred through EAP
NurseLine Free 24/7 access to a registered nurse for all employees and family members
